Perforation?: I think you are describing a perforation in the ear drum...A hole. This can occur from an infection, trauma, or even a rapid change in pressure - such as scuba diving. Small perforations often heal on their own, but for persistent pain, bleeding, etc, see an ent. Also, until it heals, protect your ear when swimming!
