It can: Gout is monoarticular and usually affects one joint per attack. 75% of the time the first metataralphalangeal joint. It is possible the secondary spot hurts cause you are altering your waking pattern. While less common, it is plausible you had two attacks, but in 16 years of practicing, I have never seen 2 simultaneous attacks.
