Red + swollen = bad: Not sure i understand question, but a surgery scar that is red and swollen sounds like it might be "infection" and that "bacteria" are present. Best to have your surgeon look at it.

May need drainage: I would discuss with your surgeon. Certain wounds such as those used for removal of a ruptured viscus are more prone to infection and may need drainage. Alternatively it could be a suture reaction.
