Check with OBGYN: Treating it as pid does not mean you have pid. They are treating you for it just in case - a positive culture for infection is the only way to make that diagnosis. In the meantime, did they do an ultrasound? Could be a cyst, or kidney stone, or even appendix. Check with your obgyn or pcp and good luck.

Hard to say: But if they didn't do an ultrasound, that would be the next step. You may have an ovarian cyst, fibroid, uti, (appendicitis would be possible, but usually shows an elevated white cell count). It is a common practice in the er setting for young reproductive age women to be diagnosed empirically with pid (without the culture results), and treated for this. It's to err on side of caution.
