Tinea Infection: Tinea cruris or tinea corporis can be a very common fungal skin infection in the breast area, especially in the skin fold under the breast. It can get very extensive with significant redness of the skin, oozing of clear fluid and extremely itchy. Using tight bras with metal lining can be very irritative and sometime causative. Sleeping w/o a bra can be beneficial. Best treat with anti-fungal rx.

Rash: I suspect you have a yeast infection on your skin. Your skin doesn't get a chance to dry out cause you wear your bra to bed. I would stop doing that. It has nothing to do with hygiene. Try some over the counter Lamisil (terbinafine) and see if that works, if not see your doctor.
