As Follows....: Send a detailed letter to your doctor listing your concerns and ask for a corrected copy of the records to be sent to you.

Deoends: There are federal record rules which all docs must follow. Each must have forms in their offices that allow you to request copies of the existing record, and forms that detail how you go about changing information contained therein. You are permitted to correct inaccuracies, but not true information you want removed. Contact the doc requesting these forms, or have your attorney do it.
