Primary care: The idea of the "gatekeeper" physician is the person who controls and limits access to the more pricey specialists and the costly imaging studies. The more the primary care physician knows and can handle himself/herself, the cheaper health care is supposed to be. In fact, cost may be increased by putting another step in the way of someone who knows they need, say, a dermatologist.
