Are you TTC?: If you're not trying to conceive, the goals of treatment are to give you regular periods or at least 4 periods a year, to reduce hyperandrogenic symptoms like acne or unwanted hair growth, and sometimes to treat Insulin resistance ('pre-diabetes'). If you're trying to conceive we're aiming to induce ovulation, as well as treating Insulin resistance, with a healthy pregnancy as the desired outcome!

Depends: There are a number of treatments for pcos that range from weight loss, to fertility drugs, to surgical management, to assisted reproductive technologies (ie: ivf, etc). The short and long term changes would be quite different and potential quite dramatic - depending on the specific treatment.
