Yes: Many children aren't attentive to their bodies at this age, particularly adhd kids. So an infection can develop & they have no pain. But often, their behavior will deteriorate indicating something's amiss. Some children have high pain thresholds, & react with minimal pain to an ear infection. If the 4yrold screams from a trivial injury, but doesn't complain of pain in a serious one, think adhd.

Certainly: The discomfort of an ear infection is primarily triggered by a stretch of the ear drum. They can have negative pressure (airplane landing) or positive pressure (pus building in ear) but when it reaches a painful level of stretch, the kid usually responds.Any slow building ear infection may not reach the painful stage.Some do quickly.If the drum ruptures, the pain often stops.

Spectrum: There is a wide range of severity and types of ear infections. Some are not associated with high pain levels. Many are found incidentally during acute visits. Please see ENT if there have been numerous middle ear infections.
