Either: Technically a "stroke" refers to an event in the brain. There are, however, different causes of strokes -some have to do directly with the blood vessels in the brain (ischemic) and some have to do with the heart or other blood vessels outside of the brain (embolic). If, for example, there is a blood clot in the heart due to arrhythmia, it can break off and go to brain causing a stroke.
