Lots of change: Throbbing is caused by dilation of blood vessels over the surface of the brain. Yet at the same time there can be spasm of blood vessels going into the brain, especially the vision and sensory areas. Chemical changes occur that affect autonomic function with nausea, vomiting, pale, sweaty, etc. Spreading depression can occur over the surface of the brain.
