No: The inability to breastfeed can be for many reasons including low milk supply, anatomy, etc... Many mothers are unable to breastfeed. This does not mean you are any less of a mother. You can still have a strong bond with your infant while bottle feeding. Also, technology has produced formula which meets baby's nutritional needs.

No to both: You are not a bad mother for not breastfeeding. Not all women can breast feed your child will not be permanently scarred for not receiving breast milk.
