Short answer: I think you need to find a second opinion quickly. One meds seem too low and the other too high. Furthermore I do not think there can be any recovery till you stop the alcohol and marijuana. Plus no sleep in 48 hrs indicates you still have serious sx.

Substance use: Your substance abuse is definitely interfering with your medications, hence lack of sufficient response even at high doses. My advise is to detox from the substances first & foremost, with the help of your doctor.
