Sure, but...: A doctor can write whatever he wants to write, and a patient can say whatever she wants to say. There are no restrictions. But, suppose the doctor did not write down something important, and he has 1000 patients. How is he going to remember what one of those patients said? If the doctor feels that something the patient said is not important, then he will most likely not write it down.
