No: Doctors (and presumably also nurse practitioners) need to have a license to practice. Malpractice liability insurance is purchased by doctors unless the insurance is too expensive, which has actually happened in some states, creating a malpractice insurance crisis . . . Meaning that the amount a doctor earns from providing patient care is not enough to pay malpractice insurance premiums.
