Lymph nodes first: Most of the time renal cell cancer is not detected in early stages, 25%- 30% cases already has metastasis at the time of diagnosis. As tumor grows invades capsule and spread to lymph nodes, then spread to liver, bone , lungs and brain.

Anywhere: Renal carcinomas can recur locally after surgery depending on the size and extent of the original tumor. A common site is lung, but it can affect liver, bone and other organs. One of the unusual sites that I have seen was a mass on the thigh the size of a football.
