Meds: Many patients do well with over the counter benzoyl peroxide or salicylic acid preparations. Some patients require topical or oral antibiotics or retinoids as well. If the over the counter meds alone don't control your acne after a month of daily use, see a doctor for additional treatment options. It is important to be persistent, and some patients "get worse before they get better".

Getting rid of acne: I tell my patients that the best one can hope for is acne control. True there are those with teenage acne that resolves in adulthood. There are also those who continue to get acne well into adulthood. Combination therapy aimed at reducing oil production, increasing skin cell turnover, reducing inflammation and controlling infection is best. Chemical peels, laser and light treatments also help.

Rx: This is the 21st century and nobody needs to have acne; life's hard enough. I start with topical Clindamycin with or without a retinoic acid derivative. If this hasn't worked well in two weeks, choose a systemic antibiotic such as Erythromycin or a tetracycline that is right for that person. Dermatologists have stronger medicines andlasers. Good luck.

Prescription Rx: If you characterize it as 'terrible', then please get with your personal physician. If you are offered isotretinoin, ignore all your friends' warnings and the scare stories online and go for it. Nothing else will help -- hygiene, diet, lifestyle changes are seldom effective.
