My 3 yr old son has had a high fever and painful constipation for the past 24 hrs. is this anything we should be concerned about?

Yes: Give him tylenol (acetaminophen) or Motrin for the fever, and some milk-of-magnesia to help with the constipation. If that is not working he needs to be seen urgently, preferably in an emergency room.
