In certain cases: Leeches inject a blood thinner into the area upon which they attach, and then suck out the blood and fluids. It turns out that is very useful for tight skin areas with limited subcutaneous tissue such as ears and over joints. There is a lively trade in medicinal leeches and once you get over the yuck factor, they can do a lot of good.
