Most cancers involve one testicle only, but those with testicular cancer appear to have abnormalities with their sperm in the unaffected testis. Removal of both testicles, chemotherapy, or retroperitoneal surgery for more advanced testis cancer can render the patient infertile.

Adopt/sperm bank: Had a similar situation show up in my practice years ago. Dad had no sperm but wanted a family. He & his wife were lucky with 2 pregnancies originating from the same donor. Both favored each other & mom. I don't know if they ever told the kids how they got here. You could also adopt, there are many that need a home.
