Macrotia surgery?: Some people have ears that simply stick out, but are normal size. This is treated otoplasty, aka ear pinning or reshaping. Some people have very large ear cartilages (macrotia) +/- large earlobes. Earlobes can be reduced by removing some tissue. To reduce macrotia is called macrotia surgery. Not routinely offered, it involves removing some cartilage under the ear rim then reattaching the rim.

Ear Reduction?: Ear reduction surgery will decrease ear size whereas otoplasty surgery will reduce ear prominence (or projection from the scalp). Very different operations with very different indications, as well as different risk/complications associated with them.

In addition...: ...There are also procedures for earlobe size reduction. We don't typically make the overall ear smaller, although that is a side effect of a reconstructive procedure that is sometimes used after a cancer excision from the ear.
