Depends: All states require a prescription for the newer insulins. Most do not require one for the older insulins or for syringes. Check with your local pharmacy for the rules in your state.

Generally, yes: Most of the insulins that are currently used in the United States require a prescriptions, although in some states the older insulins (regular and nph) can be purchased without a prescription.
