Hard to pinpoint.: Some cases of blood in the urine can be very difficult to pinpoint. Tumors, stones, and blood vessel malformations, can be notoriously difficult to localize. The most important cause not to miss is cancer, called urothelial carcinoma. Inspection of the bladder, ureters and kidneys may be necessary and muliple urine samples sent for cytologic analysis. See a urologist for this problem.
