Sometimes: This depends on how far the dementia has advanced. In the early stages of dementia, yes they can definitely know "who" is visiting -- even if their stories seem confusing or are repeated multiple times. Later, they may not seem to recognize you specifically, but they can enjoy when people show care for them. Even at the end, you still know who they are -- so visits are still worthwhile.
