What causes white cells to show in a pelvic exam? does that automatically mean i have an std or could it be something else?

Many things: White cells are part of the immune system and the immune system is active in the vagina. So the mere presence of white cells is not diagnostic for an std. Other things, like symptoms, discharge, etc come into play.
