Many things: Pain in the left arm associated with a heart attack is caused by referred pain from the heart via the nerves. Although left arm pain may be associated with a heart attack, it is not a criterion for its diagnosis. More important is the circumstances surrounding the l arm pain. Other causes include musculoskeletal - arthritis, myositis, impingement, trauma, nervous system - neck disc disease etc.
