The quantity varies, but if you are saturating more than 3 pads/ 30 minutes or having bleeding greater than 7 days, then that would be abnormal and require GYN MD evaluation. Need Gynecology Eval: Vaginal bleeding is not always your menstrual peiod. Sometimes women get miscarriages that happen about the time that they expect their periods. Also, there are a lot of conditions that can develop in the uterus that can cause bleeding such as fibroids, uterine polyps, cancer, infections, etc. Your sister should go and see her pcp or a gynecologist for an evaluation of her vaginal bleeding.

No: That is too early. Urine pregnancy tests are usually not positive until 5-6 days after you have missed a period. 2 weeks before you miss a period is right when you are ovulating.
