Many things!: Exercise. Most chronic fatigue patients are whoefully deconditioned. It makes no difference: chicken or egg. Exercise has been shown to lessen fatigue. Sleep regular hours, but do not over sleep. Finally, your doctor can prescribed low dose Nuvigil or provigil.

See details: It is a diagnosis of exclusion. There are no specific tests for cfs. Tests are done to eliminate other causes of fatigue.

Identify cause!: Poor sleep, depression, chronic viral illness, autoimmune diseases, anemia, all can cause this. The medication i favour for symptomatic relief is Provigil or nuvigil.

Many things: This is multifactorial and requires a csi like approach for diagnosis and treatment. Treatment should focus on adequate sleep patterns, hormone balance, treatment of chronic infections, rethinking nutrition and excercise.

I agree with Dr. Namey. One exercise option that can work wonders is Tai Chi. Explore the Tai Chi program from Arthritis Foundation. Tai Chi was found to be very effective in helping patients with Fibromyalgia - Wang C et al. NEJM 2010. All the best!

I should have added that Fibromyalgia has a significant Fatigue component, hence research findings for Fibromyalgia may be extrapolated to CFS.
