Depends Upon Extent: For limited and localized acne, topical therapy would be indicated using benzoyl peroxide and salicylic acid preparations. If the entire back is involved with large inflammatory painful lesions, then systemic antibiotics would be more practical. These may be prescribed by your physician.

Backs are a special case - the skin is thicker, and usually covered with clothing, and there is more oil and perspiration. Strong Rx: Consider a topical retinoic acid and/or topical benzoyl peroxide, and you will probably need at least a short course of an oral antibiotic. If you're a female, sprionolactone or an oral contraceptive pill may help. If your acne is quite severe, consider isotretinoin -- it's not for the faint-hearted but is very effective.
