Many reasons: 1) countries with higher life expectancy have more end of life diseases 2) obesity 3)record keeping, if you do not keep statistics it does not exist.

Many factors: Immunization plays a big role in eliminating many diseases that are still seen in developing countries. Availability and accessibility to better technologies and drugs (and possibly doctors) also control the severity of the diseases.
