Depends: What type of injury? Physical examination by a doctor is the first process, and determining the type and risk of injury., sometimes blood tests are indicated. Sometimes tests such as ultrasound, x-ray, or ct scan may be indicated. If the injury is penetrating such as gun or knife injury, then direct surgery may be indicated first.

Physical, X-rays: The best test to determine if you have any intra-abdominal injury is a ct scan. It is a quick and very helpful tool for diagnosing injury. This is used in conjunction with a good history and physical exam. Ultrasound is another quick test that can determine if you have fluid in your abdomen or not. Mri is another much less useful test. Finally, a diagnostic laparoscopy can also be done if needed.
