Various explanations: Seems that a character in gabriel garcia marquez novel "one hundred years of solitude" died of this. I did not read the book so can't be sure of the reference, but it might be to semmelweis' original studies on puerperal fever - rounds were at 5 o'clock. On the other hand it could refer to happy hour or quitting time as suggested by others.
