C4 protein: Complement C4 is a blood test that measures the activity of a certain protein that is part of the complement system. The complement system is a group of proteins that move freely through your bloodstream. The proteins work with your immune system and play a role in the development of inflammation. This test is usually elevated in immune diseases or infections.

Low levels = bad: Rheumatologists measure C4 and C3 levels in lupus because low levels are seen in active disease.

C4 low?: C4 deficiency is commonly associated with sle. Therefore in this case it does not measure activity. To assess lupus activity, other than clinical findings, i always get a c2, c4, and a c3.
