PSA does not rise.: After prostate surgery, the psa should fall to undetectable levels. An undetectable psa is the measure of success, and a rise in psa signals local or distant recurrence. After radiation treatment, the psa falls to a lowpoint, or nadir, and should stay low. The same goes for metastatic cancer, where hormonal therapy should keep the psa from rising.
