Inject or ultrasound: Injection with depot steroid (if not doing heavy lifting) or ultrasound - if the diagnosis is accurate.

Physical therapy: Physical therapy will cure shoulder bursitis in most cases. A cortisone shot into the bursa can also be curative. For people who do not respond to those, i will usually get an MRI to determine if there are other problems such as a rotator cuff tear. Not all tears need surgery however.

Other way around bes: I never send a patient to pt first. Very expensive. Need a thorough examination, then nsaids with injection of corticosteroids. If relief is short-lived, than rotator cuff issues become likely. Pt is then used if relief is not optimal. This is the most cost-effective route.
