Sepsis: Dishcarge planning after neonatal sepsis may include routine newborn care like state newborn screen, hearing screen, and potential vaccinations. Health care team should speak to the pediatrician, provide prescriptions/equipment, discuss parental concerns/expectations/train for care/development and recommendations for other follow up and therapies (ie speech, physical or occupational, if needed).

DC planning: Any discharge planning should include, follow up with primary care doctor, appropriate specialist, medications, antibiotics if necessary, oxygen or any other dme equipment, diet, nutritional requirements, activity eyc.
