Many things: Sometimes eyes appear prominent because they are genetically larger or set in small sockets, sometimes because they are pushed forward by swelling behind the eye as in grave's disease, tumor, myopathy, histiocytosis, myasthenia gravis or cortisol excess. Sometimes a lid retraction or decreased blink can cause the same appearance.
