Need two: True depth perception usually requires 2 eyes (same for 3d at the movies). People who only have vision in one eye compensate by being a good judge of size, etc.

Yes: Depth perception dysfunction is usually due to childhood strabismus, or poor vision in one eye. These things are usually permanent and cannot be treated. Blindness in one eye prevents simultaneous binocular vision which ends true depth perception (stereovision).
