The rate of change: The rate of change of the blood sugar can affect the heart rate and blood pressure through the sympathetic nervous system output.

Not directly : There is no direct effect other than for the stress induced by a hypoglycemic reaction (hr and BP would go up) or the diuretic/dehydrating effect of prolonged hyperglycemia (bp would go down while hr went up).
