Casodex (bicalutamide): Va (veterans administration) will typically cover Casodex in patients who have a rising psa after Lupron (leuprolide) injections--an oncologist a va has to be the one who orders this (non-va doctors can not order drugs at the va).

Yes: Bicalutamide is standard for men with prostate cancer progressing after initial castrating therapy (medical or surgical). Some doctors prefer to use it with initial therapy (dual blockade) while others use it sequentially. Details are here: http://bit.Ly/loybx2.
