Clean area well don': Make sure tetanus is up to date. Need to be concerned about infection given the deep penetration and may likely need antibiotics to prevent infection.

Aggressively: At the very least your tetanus status should be up to date. Additional treatment varies depending on the degree of contamination. Deep and dirty wounds will require antibiotics as well as a possible incision and drainage procedure. X-rays or imaging may also be warranted after a thorough physical examination for possible neurovascular injury.
