Medicare/medicaid: Medicare is a federal medical insurance for people over the age of 65 and people on disability, and people with end stage renal disease and is run by federal government. Medicaid is run by state governments and is funded in part by federal govt, it is medical insurance programme for low income families, it is managed by states, and hitorically pays much less for doctors and many doctors dont participa.

All are complex programs with lots of rules and regulations of which only a fraction of physicians even know about.
