Disease vs symptom: Coronary artery disease is cholesterol build up in the coronary arteries. CHF is a symptom complex of shortness of breath . Unable to lay flat . Waking up short of breath and decrease exercize tolerence. It usually occurs when blood backs up in the lungs as a result of the heart not working efficiently. CHF is not a diagnosis as there are many causes and a source should be found.

CAD/CHF: Coronary artery disease is narrowing of the arteries to heart. Congestive heart failure is weak pumping action of the heart. Weak pump can be due to coronary artery disease. There are other causes of weak pump causing congestive heart failure.

CAD CHF: CAD is narrowed arteries of the heart, may cause heart attack, which may cause heart to get weak & not pump well causing heart failure. In heart failure you retain fluid and makes it hard to you to function (get fatigue easily, may get another heart attack or shortness of breath. Diabetes makes you more prone to have CAD and chf.

Yes: Coronary artery disease means the arteries of the heart have partial or complete blockage. Congestive heart failure means the heart musle is weak and not pumping blood well enough and fluid back up into the lungs. Often patients with congestive heart failure have coronary artery disease, but not always.
