Depends: If it is solid in ultrasound a biopsy can be done. This can be done with a needle. If it is growing, might as well remove it. If it is symptomatic remove it.

Several steps: The 1st step is to see if it is overproducing thyroid hormone(< 5% do). If so, it is treated with surgery or radioactive iodine. If not, the next step is to determine if it is cancerous(~ 5% are). If so, the next step is surgery. If not cancerous, it can be followed unless it is so large that it is causing trouble swallowing, hoarseness, or uncomfortable pressure in the neck; if so, surgery.

Variable: Management depends on a number of factors like if the patient is hypothyroid or hypothyroid at the same time or their thyroid function is normal. Also depends on the size of the nodule and characteristics on ultrasound. Lastly whether the patient has a single nodule or a multinodular thyroid gland matters. That is why a thyroid specialist should always see you if a nodule is discovered.
