I am 27, a testicle almost twice the other (it's hard). i don't remember they were ever the same size. haven't yet taken ultrasound. can it be cancer?

Yes: Yes, this could be testicular cancer. If so, the sonogram and blood tests will raise the suspicion, but the diagnosis is secured only by surgical removal. If doctor strongly suspects cancer, he will not recommend a needle biopsy across the scrotum , but rather removal via a pubic incision. Testicular cancer is the most common cancer in men your age, but it has high cure rate, even if metastatic.
