Menses: Sometimes when a woman is on her period she will notice tissue on her pad, tampon or even in the toilet. This is usually the lining of the uterus called the endometrium. This is why you have a period monthly to release the endometrium if you do not have a fertilized egg to implant in the endometrium. If you are concerned about pregnancy I suggest you check a pregnancy test.

Bleeding: Check a pregnancy test as what you described could be miscarriage tissue. IF you pass anymore, you might want to save it and put it in a ziplock bag so your doctor can send it off for pathology evaluation. This could also be membranous endometrium or mucus. Hard to know without seeing the tissue.
