Not definitely known: Heredity is definitely a factor with Scheuermann's disease. Other than that, the cause is essentially unknown, with various theories such as mechanical, metabolic, and endocrine causes.

Not known for sure: Scheuermann juvenile kyphosis or Scheuermann disease is most frequent cause of kyphosis in adolescence. However, natural history and genetic basis is still unknown. Reports of identical radiological changes in monozygotic twins, siblings, and transmission over three generations suggest underlying heritability.
