High BMI: Most insurance companies provide bariatric surgery coverage for patients with a bmi (body mass index) > 40, or a bmi of 35 with certain comorbidities. Your doctor should know your bmi, or online bmi calculator is widely available.

Morbidly obese : Bariatric surgery is indicated for patients who have a body mass index (bmi) over 40, or over 35 with weight-related medical problems like type 2 diabetes, high blood pressure or cholesterol, sleep apnea to name a few. Bmi is determined as shown on the attached chart.
