More Going On: I'm sorry that he is so sick. This does not sound like it is caused by his face, but that he is having some swelling from being so sick. That is only an assumption on my part. What you need to do is get with his family and discuss with them what is going on. In today's world of medical privacy it is difficult for non-family to get information. I hope he does well.

Not face tumor: malignant tumors of the face, that is of skin, when spreading, do involve lymph nodes of neck or go to lung but do not cause facial swelling. This is from output failure from veins and lymphatics draining face and most likely due to a superior vena cava syndrome due to blockage of main vein in chest. This requires his need for respirator secondary to blockage of airway from facial swelling.
