Needs evaluation: This requires an exam and testing. Genital herpes needs to excluded as a cause. Other skin disorders such as lichen sclerosis and lichen simplex chronicus are possibilities. Even recurrent yeast can be a cause.

Low estrogen: This can be due to thinning of the lining of the vagina from low estrogen levels or small tears or dryness. I would use estrogen vaginal cream around the opening twice a day. Can try other lubricants such as coconut oil or vasoline. Occasionally testosterone cream is used.
